Chapter I
 
General provisions
 
§ 1
 
Individual Association for Home Care and Social Assistance "FIDES" is a voluntary, self-governing association of people who want to work for persons with disabilities, particularly for chronically ill persons with disabilities not eligible for hospitalization and requiring continuing care services.
 
§ 2
 
Association for Individual Faraway Home Care and Social Assistance "FIDES" in the following provisions of the statute is called the Association. Association, adequate to the needs, can use the abbreviated nazwyStowarzyszenie "FIDES"
 
§ 3
 
The area of ​​operation of the Association is the Polish Republic, especially the city of Lublin. The seat of government is the city of Lublin, where is the Board of the Association.
 
§ 4

The Association may be a member of national and international associations of the same or related field of action.
 
§ 6

§ 8

Chapter II
 
Objectives and means of action
 
§ 9
 
The aim of the Association is:
 
1. increase the activity and resourcefulness of people living with disabilities,
 
2. increase the efficiency of care of the family,
 
3. improve the quality of health care for the chronically ill and people with disabilities, promoting the idea of ​​helping people, the fight against social exclusion, equal opportunities in access to information for civil society.
 
4. preparation and serving of meals and the distribution of food for the poor.
 
 
 
The specific objectives of the Association are:
 
a) professional, care and care of the chronically ill and disabled, understood as: professional care and comprehensive care and continuation of drug therapy and diet.
 
b) to supply people with mild mental support tailored to the specific needs of these people
 
c) to assist the families of those persons and continuous cooperation with them,
 
d) conducting various forms of care and activities
 
e) permanent cooperation with the institutions of the Ministry of Health, Welfare institutions, associations and organizations of a similar nature,
 
f) health education in the field of care for patients with physical or permanently immobilized.
 
§ 10
 
The Association uses the following means of action:
 
 
 
I. activities free of Association:
 
 
 
1. PKD 88.10.Z. Social work activities without accommodation for the elderly and disabled, including visiting the elderly and sick in their homes
 
2. PKD 91.33.Z. Promoting the idea and methods of care for people, including mental disorders, primarily located in a difficult situation or not having proper environmental conditions during illness
 
3. PKD 85.59.B Other education nec, including: conducting training of medical personnel and other employees of a comprehensive long-term care, rehabilitation and patient care at his place of residence
 
4. PKD 88.42.B. Provision of services nursing - care in the patient's home, including bringing the individual help in creating the most favorable conditions for the care of these patients, including the adaptation of the apartment.

§ 11

Members - rights and obligations
 
§ 12
 
Members of the Association are divided into:
 
• ordinary
 
• support
 
• honorary
 
§ 13
Ordinary members are individuals, with full legal capacity, which indicate their adherence to the Association and, due to its ethical values ​​- moral and professional will be adopted by resolution of the Board of the Association.
 
§ 14
 
Ordinary members have the right to:
 
• participate in the General Assembly of the Association of active and passive suffrage,
 
• participate in the work of the Association to use the seal.
 
• submit proposals and requests to the authorities of the Association.
 
§ 15
 
The duties of the ordinary members are:
 
• compliance with the statute and objectives of its provisions,
 
• conscientiously implement the resolutions of the Association,

• regularly pay membership fees.
 
§ 16
Supporting members may be individuals with full legal capacity and legal persons that work by the plenipotentiary.
 
§ 17
 
Supporting members have the right to:
 
• submit proposals and requests to the authorities of the Association,
 
• except for legal persons - the right to vote at the General Assembly.
 
§ 18
 
Supporting members are obliged to:
 
• observe the provisions of the statute,
 
• assist the Association in carrying out its statutory tasks and regularly pay membership fees.
 
§ 19
Honorary members of the Association may be individuals who gave such dignity at the request of the Board-General Assembly. The dignity of this can be attributed to persons of particular merit in pursuing the statutory. Honorary members have the right to participate in the work of the Association, they have no right to use the seal, can submit proposals and requests to the authorities of the Association. Honorary members do not have voting rights. Honorary members are obliged to abide by the statute of the Association and regularly pay membership fees.
 
§ 20
 
Membership expires as a result:
 
• the death of a person or loss of legal personality by a legal person,
 
• exclusion of a member of the Association at the request of the Board in connection with an attitude inconsistent with the provisions of the statute. Since the decision to exclude the member has the right to appeal to the next General Assembly, which will make the final decision; the appeal must be submitted to the Management Board within 30 days from the date of notification of exclusion,
 
• resignation in writing
 
• dissolution of the Association.
 
Chapter IV
 
authorities Association
 
§ 21
 
The governing bodies of the Association are:
 
• The General Meeting of Members,
 
• Board of Directors,
 
• Revision Committee.
 
§ 22
 
1. The term of authorities is 5 years
 
2. Resolutions are passed by a majority of votes in the presence of at least half of the members entitled to vote, unless otherwise specified in the statute. In case of equality of votes, the vote of the chairman of the meeting.
 
3. In case of resignation, exclusion or death of any member of the Association during the term of office, the composition of these authorities is complemented reported from a non-selected candidates in order of votes obtained. GENERAL MEETING

MANAGEMENT ASSOCIATION
 
§ 28
 
1. The Management - manages all the activities of the Association in accordance with the resolutions of the General Meeting of Members
 
2. The Association Board consists of 5 persons:
 
• President
 
• Vice President
 
• Treasurer
 
• Secretary
 
• Member of the Board.
 
3. Members of the Board are elected by the General Assembly from among its members.
 
4. On his first election meeting The Board elects the president, vice president, secretary and treasurer.
 
5. For his work and activities The Board is accountable to the General Assembly.
 
6. Members of the Board of the Association may be employed in the Association both in management positions and lower level, while their salary can not be higher than 2 times the monthly salary in enterprise sector announced by the President of the Central Statistical Office for the previous year.
 
§ 29
The powers of the Board include:
 
• representing the Association outside,
 
• implementing the resolutions of the General Assembly,
 
• directing all activities of the Association,
 
• adoption of the draft budget of the Association and the project membership fees,
 
• managing the assets of the Association and its financial
 
• reporting to the General Assembly on the activities of the Board,
 
• initiating and programming activities of the Association,
 
• organization of General Meetings,
 
• accepting members, keeping their records, collection of contributions declared, release members from the obligation to pay membership fees,
 
• appointment and dismissal of employees,
 
• deciding on the amount of the membership fee
 
§ 30
If necessary, the Management Board may establish committees. Members of the Commission may be persons outside of the Board, but its chairman is a member of the Board.
 
§ 31
Board meetings should be held at least once every four months at the request of the President of the Management Board.
With meetings of the Board Secretary shall prepare a report, signed by chairman of the meeting.
 
§ 32
Board meeting should be convened at the request of the Audit Commission, within 30 days from the date of application.
 
§ 33
 
1. President - represents the Association and directs its activities in the period between the meetings of the Board.
 
2. The President of the Management Board and consists of the Audit Commission report on the decisions
and implementation of the resolutions.
 
3. The President of the General Assembly consists of a report on the activities of the Association during the term.

§ 34

§ 38
 
1. The Audit Committee carried out at least once a year, control of the whole activities of the Association with particular emphasis on financial management and assets in terms of expediency, legality and savings and supervises the activities of the Board for compliance with the statute and program of action. It shall submit a written report on the inspection, and if appropriate, proposals and deadlines to remedy deficiencies.
 
2. The report is submitted to the General Assembly with a request to grant or not to grant discharge to the outgoing Board.
 
§ 39
 
The Chairman of the Audit Committee or its designated member of the Commission may participate in the meeting of the Board in an advisory capacity.
 
§ 40
 
The Audit Committee is a body distinct from the governing body and not subject to him in the performance of internal control or supervision, the members of the control body:
 
a) may not be members of the governing body or remain with them married,
in cohabitation, in consanguinity, affinity or subordination,
 
b) they have not been convicted for an offense prosecuted by indictment or a fiscal offense,
 
c) may receive for performing their functions in the body reimbursement of reasonable costs
or compensation in an amount not higher than the average monthly salary in enterprise sector announced by the President of the Central Statistical Office for the previous year;
 
 
 
Chapter V
 
Assets of the Association
 
§ 41
 
1. Assets managed by the Board of the Association.
 
2. The funds of the Association consist of:
 
• Income from membership fees,
 
• Records, legacies and donations
 
• Subsidies and other social influences
 
• Income from your own business, subject to § 4 7pkt
 
§ 42
 
The Association shall keep accounts according to applicable regulations. Procedure and forms of accounting and reporting established by the Board of the Association.
 
§ 43

c) the use of the property for the benefit of members, members of governing bodies or employees and their relatives in a manner other than in relation to third parties, unless such use directly stems from the statutory,
 
d) purchase of goods or services from entities in which members of the organization, members of its bodies or employees and their relatives in a manner other than in relation to third parties, or at prices higher than the market.
 
Chapter VI
 
Changing the statutes and dissolution of the Association
 
§ 45
 
Changing the statutes or dissolution of the Association requires a resolution of the General Assembly adopted by a majority of 2/3 of votes entitled to vote in the presence of at least 1/2 of the members of the Association.
 
§ 46
 
The General Assembly appropriate resolution specifies the mode of dissolution of the Association, the manner of liquidation and the purpose for which will be the transfer of assets. In the event of dissolution of the Association General Meeting appointed liquidation committee.

In matters not covered by this statute, the provisions of the Act - Law on Associations.
